Simple instructions for your Vote By Mail Application

• Fill in or correct your personal information at the top of the application.
• Check the “General ” box to request a vote by mail ballot for the November 2010 election.
• You do not need to choose an Vote-By-Mail Option. This can be left blank if neither option applies to you.
• Fill in the correct mailing address where you would like to receive your ballot. If this is the same address at the top of the page, you can leave this section blank.
• Please remember to sign and date your ballot application where indicated underneath the mailing information. Your application will not be accepted without a signature.
• Fill out the return address in the return address portion in the upper left-hand corner on the back side of the application.
• You do not need to fold the application, simply place a stamp in designated location and drop it in the mail.

If you applied for an Vote-By-Mail ballot but never received it, you can vote by provisional ballot at your polling place on Election Day.

Mailing in your application - deadline is October 26th.   

A voter may apply for an Vote-By-Mail by completing an Vote-By-Mail Application (click here to download) and mailing the application to their County Clerk by mail by October 26th. A voter may also apply in person to the County Clerk until 3:00 PM, the day before the election. The County Clerk cannot accept faxed copies of an Vote-By-Mail Application since an original signature is required.
